Home Care Field Registered Nurse (RN)-BURLINGTON COUNTY

Where: Visiting Angels Registered Nurse (RN) to visit client in their homes throughout Burlington County

Shift Type: Per Diem

Compensation: $70/visit for initial assessment and $60/visit for 60-day visits.

Visiting Angels is seeking a New Jersey licensed Registered Nurse (RN) for in-home visits to our clients throughout Burlington County and on occasion some Mercer County clients, as well (optional). After applying, please complete the brief screening questions to be considered.

• This is a non-medical per diem position
• There are no set hours for this position

Our Field RNs prepare thorough and accurate Initial Assessments and Reassessments and develop personalized care plans. This per diem position is ideal for an RN who desires a significant level of work but with flexible hours. Most visits to our clients occur on weekdays.

Home Care Field RN responsibilities include:

• Setting appointments with Visiting Angels' clients in their Burlington County homes during a regular Visiting Angels' care shift; mileage is reimbursed per Company policy
• Creating in-home initial client assessments and care plans based on clients' specific care needs
• Conducting follow-up in-home assessments at specified intervals
• Conducting periodic telephone assessments
• Revising care plans as necessary
• Case management
• Supervising Certified Home Health Aides (CHHAs)
• Performing Orientations/Skills Assessments of CHHAs

Qualifications include:

• Prior Home Care RN experience STRONGLY preferred
• Current New Jersey Registered Nurse license
• Ideal candidate resides in Burlington County
• Enjoys meeting with and talking with older adults
• Prior RN experience with in-home care assessments
• Case Management experience
• Excellent written and verbal communicator with strong interpersonal skills
• Must be self-motivated, organized, and able to work independently
• Able and willing to travel to clients' homes throughout Burlington County
• Has a valid driver's license and a reliable car, willing to travel to clients' homes
• Has basic computer skills (data entry on a web-based program, email, scanning)
• Has weekday flexibility
