Job duties would include but not limited to coordination of care under the direct supervision of a RN DON.

JOB KNOWLEDGE AND ROLE RESPONSIBILITIES:

  • Demonstrates knowledge of developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning the delivery of care.
  • Demonstrates knowledge, skills, and techniques necessary to care for residents with the following needs: therapeutic, neuro and dementia, dialysis, IV therapy, infectious disease, and wound care, if applicable.
  • Acts appropriately under the direction of the DON and acts as an active member of the interdisciplinary team.
  • Demonstrates ability to adjust to changes in unit/shift assignments to meet resident and facility needs.
  • Ensures the residents receive skilled nursing and rehabilitation services.

Wears appropriate name badge which identifies self as a CNA. • Accurately collects and records patient data (vital signs, intake and output, blood sugar, weights, restraint care/monitoring, other) and complies with hospital documentation standards to ensure a complete and accurate patient record. • Initiates a prompt response to all patient call lights and seeks assistance from co-workers as appropriate. • Assists with admission, discharge, and transfer of patients as appropriate for the inpatient or ambulatory area patients.
